Occupational Therapist/Physiotherapist - Case Manager

- Work without general supervision to provide case management service to a select population of clients and families experiencing brittle support systems, family conflict, social, cognitive and/or physical functioning decline, and/or mood and behaviour issues.
- Conduct assessments that considers psychological factors and the surrounding social environment on physical and mental wellness and their ability to function in home and community care.
- Provide individual and family/caregiver support, make recommendations about care, maintain client records, and attend interdisciplinary clinical/case conferences.
- Identify applicable community resources for clients, contribute to the effectiveness and development of community resources and planning with clients for care and support in accordance with their health needs.
- Provide continuity of care across the continuum by liaising with team members, develop/evaluate care plans and coordinating, implement clinical interventions, evaluate client outcomes based on client goals for care, coordinate ongoing care and supporting transition.
- Consult, confer, and collaborate with multidisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in client-centred care and education.
- Current full registration with either the College of Occupational Therapists of British Columbia (COTBC) or the College of Physical Therapists of BC (CPTBC).
- Two (2) years’ recent experience working with geriatric clients and/or adults with chronic illness in rehabilitation or community settings, or acute settings or an equivalent combination of education, training and experience.
- Eligible for membership in the Canadian Association of Occupational Therapists (CAOT) or the Physiotherapy Association of British Columbia (PABC).
- Valid BC Driver’s License. Local area travel requires the use of personal vehicle.
- Comprehensive knowledge and demonstrated ability to apply case management principles.
- Broad knowledge of case management and practice within a client and family centred care model for clients with complex health care needs.
- Broad knowledge of relevant legislation that governs care for vulnerable adults such as adult guardianship act, mental health act and admissions to care facilities act.
- Demonstrated ability to apply systems theory to practice in Occupational Therapy or Physiotherapy assessment and intervention.
- Demonstrated ability to conduct assessments of physical, cognitive, psychological, social, functional and environmental factors.
- Demonstrated ability to apply a psychosocial approach to identify influences on physical and mental wellness and ability to function in home and community.
- Demonstrated an understanding of Trauma Informed Practice, Harm Reduction and Recovery Oriented Care.
- Demonstrated ability to promote Indigenous Cultural Safety
- Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations
- Demonstrated ability to utilize conflict resolution and therapeutic communication.
- Comprehensive knowledge of evidence based practice related to care of clients needing supportive case management.
- Comprehensive knowledge of other health disciplines and their role in client care.
- Comprehensive knowledge of adult education principles, methods and tools.
- Demonstrates ability to problem solve using sound judgement in applying critical thinking skills within safe limits of client care.
- Demonstrated ability to provide leadership and work direction to the supporting clinical team
- Demonstrated ability to teach, demonstrate, coach and facilitate clients and families.
- Demonstrated ability to develop and implement project action plans and accomplish objectives.
- Demonstrated ability to work independently.
- Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
