- To provide assessment and support to inpatient mental health individuals.
- To provide individual and family assessments to determine the type of services required for successful discharge
- To provide supportive counseling to individuals and their families.
- Provide education to individuals and their families regarding mental health.
- To provide appropriate referral to community supports to ensure the health and wellbeing of individuals post discharge.
- To function as a member of a multi-disciplinary team which provides assessment, education, supportive counselling, treatment and discharge planning support to patients and families on the inpatient mental health unit.
- Function as a member of the interdisciplinary team, by participating in treatment rounds, staff meetings, case conferences and family meetings
- Perform individual, couple and family assessments
- Provide short-term, supportive counseling and education
- Support clients with the discharge planning process
- Facilitate applications for placement/housing, income support programs, and referrals for community resources
- Plan, lead and facilitate psychoeducational and life skills groups
- Document assessments, treatment plans, clinical progress notes, etc.
- Based on assessment of patient need, provide referrals to appropriate community resources for the purposes of discharge planning
- Facilitate daily inpatient rounds with participating community partners
- Liaise with community partners as required to discuss program changes, patient care needs and to promote good working relationships
- Support the patients and the treatment team through group programming
- Participate in the collection of statistical data (RAI)
- Ensure appropriate support services are in place when patients are discharged
- Participate in committees as deemed appropriate, such as unit council, SW professional practice council, etc.
- Provide supervision and observational opportunities for students completing field placements
- Masters in Social Work (MSW)
- Three to five years' experience working with seriously mentally ill clients.
- Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Advanced counselling and assessment skills with various short-term counselling modalities, including Brief Solution focused model, cognitive behavioural and dialectical behavioural therapy
- Demonstrated ability to work collaboratively in an inter-professional team environment as well as independently.
- He/she role models both professional and the organizational core competencies.
- Ability to de-escalate a crisis situation
- Good Working knowledge of Social Assistance forms
- Knowledge of clinical practices
- Knowledge of hospital informational systems, B Care, HPF, RM&R, Microsoft Office
- Advanced group facilitation skills
- Advanced assessment skills
- Ability to work within an interdisciplinary team
- Knowledge of community resources, income support programs and housing programs
- Ability to work with individuals and their families
- Advanced Counseling Skills, i.e. CBT, Solution Focused, Suicide Prevention Skills
- SMG Training
- Understanding of the Mental Health Act
- Understanding of Mental Health Disorders
- Good working knowledge of Social Assistance programs/forms
- Demonstrated excellence in communication, both written and verbal
- Demonstrated analytical and problem assessment skills
