To help fulfill its mission, LOFT is hiring a Case Manager.Job SummaryThe Case Manager provides a range of services that supports individuals living with serious mental illness, addictions and physical health issues in York Region and South Simcoe. The Case Manager’s goal is to empower clients, thereby increasing their independence, dignity and self-respect in a safe and secure environment. This position liaises with other community agencies and professional services.The York Region/South Simcoe Community Case Management Program provides short-term support or intensive case management for transitional age youth, adults and older adults (also associated with LOFT’s Integrated Psycho-geriatric Outreach Program). We offer one-to-one support to assist individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life.What You Will DoDevelop a plan of support and recovery, which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCAN’s.Promote the personal empowerment of each individual through coaching, mentoring and support.Directly assist with their daily living activities, particularly in the acquisition of life skills such as hygiene, medication management, cooking, cleaning, shopping and budgeting.Assist individuals with enhancing interpersonal skills as neededEstablish and co-ordinate a support network of social, family, medical, psychiatric and other community links. Provide crisis intervention/management and assist in the development of crisis plans with clients. Support program administration by completing and maintaining the required records, assessments or statistics such as client files.What You BringMust HavesCompletion of a degree/diploma in Social Work/Human Services or equivalent.Minimum of three years’ experience in providing support to vulnerable adults living with serious mental health issues and/or concurrent disorders and/or justice involvement and/or complex needs.Ability to directly assist residents with daily living activities, promoting the acquisition of life skills.Understanding of psychosocial rehabilitation and recovery principles, harm reduction, and the therapeutic relationship and the ability to apply these concepts into practice.Knowledge of issues faced by adults with serious mental illness and addictions and of available community and other professional services in York Region/South Simcoe.Creativity and flexibility to approaches.Good oral and written communication skills, counselling and responding skills to engage clients in highly personal areas.Ability to work effectively within a team environment and to interact with workers from a variety of agencies.Ability to implement assessment tools, develop and implement care plans in conjunction with client.Crisis intervention skills, with well-developed problem solving abilities.Possession of a valid driver’s license use of a reliable vehicle is required and insurance to transport clients.Nice-to-HavesUnderstanding of cultural competency and anti-oppression frameworkReflective of a multi-barriered group, as serviced by the program, a strong asset What We OfferE-learning program available all year round Paid vacation, personal and sick days Access to unlimited DEI trainings through partnership Canadian Centre for Diversity and InclusionA chance to push your ideas from inception to executionSatisfying work with a fun, energetic and supportive leaderAn opportunity to be a change maker in an organization that reaches people that are often overlookedOur Commitment to Diversity and InclusionLOFT Community Services is a values-driven organization with a deep belief that each person has the ability to grow and change.
