We currently have an opportunity for a Clinical Manager, Transitional Care Unit in the Complex Continuing Care (CCC) Program at Freeport Campus.

How the Clinical Manager in the Transitional Care Unit support our teams at Grand River Hospital:

Grand River Hospital's Transitional Care Unit Inpatient Program is available at the Freeport Campus. Reporting to the Program Director, the Clinical Manager is a highly motivated, resilient and an adaptable professional, with advanced clinical and critical thinking skills.

Success in the role requires you to demonstrate an ability to foster a culture of compassion through caring relationships, leading practices for patients transitioning to alternate services and partnerships. You are a strong leader with extensive experience leading in a busy and transformation environment. You will guide and support the strategic and operational planning including patient care delivery, access and flow, clinical effectives and efficiency, human resources and financial planning.

At Grand River Hospital, the Clinical Manager has:

  • Bachelor Degree in a Health Profession required, preferably nursing, with current registration in good standing with the Regulatory College of your discipline required;
  • Preference will be given to candidates with clinical working experience with a senior population, within a health care environment;
  • 3-5 years of previous experience in a leadership role in a healthcare environment.

Demonstrated excellence in:

  • Strong interpersonal and leadership skills and abilities, including excellent communication and organizational skills, conflict resolution, change management, critical thinking, and problem-solving;
  • You have demonstrated expertise in partnerships with acute care hospital/community sector and an understanding of transitional issues between health care environments;
  • Solid ability to manage a diversified workload with rapidly changing priorities with an ability to multi-task;
  • Coordinate and be accountable for the day-to-day operations and utilization of resources to achieve the service's goals;
  • Demonstrated ability to mentor, support, and facilitate a healthy workplace environment that fosters excellence in care;
  • Demonstrated ability to create an environment of continuous learning; engaging staff, patient and families in continuous improvement
  • Quality management and program evaluation;
  • Knowledge of best practices, standards of practice, and relevant legislation governing health activities and Long Term Care in Ontario;
  • Advanced written skills
  • Financial acumen and human resource management;
  • Computer proficiency (Microsoft word, e-mail, internet etc.);
