Job Description Under the direction of the Manager, the Assessor Coordinator - Degree with Early Childhood Speech Therapy Team. As a member of the Early Childhood SLP Team, the Assessor Coordinator - Degree provides intake and system navigation to clients and to agencies, by providing screening, assessment, consultation, data collection, organizing and communicating referrals, as well as family and community education. The Assessor Coordinator - Degree acts as a liaison and advocate for clients/patients, ensuring that clients with the greatest need have access to service in a timely manner. The incumbent will have a strong demonstrated knowledge of the social determinants of health, and their impact on health outcomes. The incumbent will work with Primary Health care providers including, but not limited to, speech language pathologists, nursing, psychology, social work, community program builders, Nurse Practitioners, and senior staff to triage referrals. Bachelor degree in Occupational Therapy, Physical Therapy, Speech Language Pathology, Social Work, or related discipline Registered or eligible for registration with the appropriate licensing body Valid Driver's License
Knowledge, Skills and Abilities Ability to establish and maintain effective working relationships with a wide variety of professionals, public agencies and families Ability to exercise good judgement, critical thinking and problem solving in decision making Ability to plan, organize and coordinate efforts with other multidisciplinary team members as well as communicate effectively and adapt to changing conditions Ability to work under pressure, meet deadlines, and manage time and stress; requires strong coping skills Demonstrate resourcefulness, ability to work independently and in collaboration with others as required Demonstrated skill in advocacy, negotiation and conflict resolution Demonstrated skills with client interviewing, assessment, triage, facilitation and counselling Demonstrates cultural competency and the ability to work with diverse groups Excellent interpersonal, verbal and written communication skills Knowledge of local human service organizations, community resources, referral procedures and the ability to research alternative care options Previous experience in case management in a health care setting with vulnerable children and families is an asset Sound working knowledge of Health Information Protection Act (HIPA), Child & Family Services Act, and related legislation Understanding of health promotion and health prevention trauma informed care and health equity approaches
