Job Description The Assessor Coordinator - Degree Senior is accountable for assisting the Home Care Manager to ensure efficient and effective coordination and delivery of home care services. This position ensures a high quality of client care in keeping with the philosophy, objective, policies, and procedures established by the HHR. The Assessor Coordinator - Degree Senior through a collaborative case management process is accountable to ensure that the client has access to appropriate, adequate, high quality, coordinated and integrated service that is continuous throughout the healthcare agencies (system.) The Assessor Coordinator - Degree Senior must utilize a collaborative client centered process that respects the client's dignity, the client's right to make informed decisions and the role of family, caregiver's and community resources in planning care and caring for clients. The Assessor Coordinator - Degree Senior promotes quality care and cost effective outcomes while addressing the health and well being of the client based on the client's unmet needs by assessing, planning, coordinating, implementing, monitoring and evaluating services received by the client. This position has staff supervisory responsibilities. Job Qualifications Required Qualifications
Bachelor Degree in Nursing or Bachelor Degree in Social Work or Bachelor Degree in Physiotherapy or Bachelor Degree in Occupational Therapy Registered or eligible for registration with the appropriate licensing body Valid Class 5 driver's license
