McMan is recruiting for a Case Manager to join our Hope Homes program. Operating from a Housing First and Harm Reduction Approach; the Case Manager will be responsible for assisting youth in obtaining and maintaining safe housing, supporting their education or employment goals and any other areas of support that is required to help them successfully transition out of homelessness and into independence.
We are looking for someone who has a passion for working with youth who are experiencing or at risk of becoming homeless, and one who shares McMan’s core values of Respect, Trust, Empathy, Genuineness and Commitment.
Some of the key aspects of this role include:
· Meet client, program and organizational needs while working from a flexible schedule.
· Provide comprehensive community resource information to youth based on the individual needs.
· Educate and support youth to increase skills that promote greater self-efficacy.
· Meet with the youth in their home or community environment.
· Provide individual instruction, modeling and support of basic life skills (ie: health, nutrition, home cleanliness, safety).
· Provide support in the areas of addition, mental health, and education or employment.
· Monitor and support youth through a harm reduction approach to ensure living conditions are safe and sanitary.
The successful candidate has:
- A degree or diploma in Human Services.- Knowledge of youth homelessness, community resources.- Experience with HMIS data would be preferred.- Three (3) years’ experience working with youth in case management.- Excellent organizational and communication skills.- Strong writing, organizational, program planning and interpersonal skills- Demonstrated interest and knowledge of Indigenous history, language and culture would be an asset.- Current standard First Aid Certification and Suicide Intervention Training.- Valid Driver’s license and adequate insurance coverage.- The successful candidate must also have a Child Welfare Intervention Check and Police Check (current within 6 months).- Knowledge and experience with the Relational Theory, harm reduction and stages of change is an asset.
*This position is required to participate in on-call rotation of one week 24 hour support for the Agency.

Salary Range: $49,768 - $54,931 per annum
